Fill in your affiliate account information as follows:

1. Click on "My Account" in the left hand frame My account
2. Click on "Edit Tab" across the top
3. Click on "Affiliate/Personal Info" tab
4. Fill in first name, last name, phone number, click the box "SSN/TIN is for my company if applicable, enter your SSN/TIN in the box, fill in your address
5. When complete click the "Save" button
